Vancomycin is the drug of choice for the treatment of methicillin and related beta lactam antibiotic resistant Staphylococcus aureus (MRSA) as well as for the treatment of serious gram-positive infections where allergies to penicillin or cephalosporin play a role. Monitoring of peak and trough serum levels is necessary due to the potentially serious side effects including: ototoxicity, nephrotoxicity, phlebitis, and reversible neutropaenia.
Post dose levels are NOT necessary.
Pre dose: 10-20 mg/L
