Help With Request Form Completion

The following data sets must be present on the request form for correct identification of patients:
- Patient NHS Number
- Patient District Number
- Patient Surname
- Patient Forename
- Patient DOB
- Requesting Clinician
- Request Location
The following information should be supplied for correct investigation and relevant clinical interpretation.
- Specific assay required
- Clinical details / relevant clinical history (particularly important in allergy diagnosis).

Handwritten GP Request Forms
- Handwritten GP request forms must be completed with the same identification data as above. Unfortunately, there is currently no separate box for Immunology tests, therefore these should be clearly written in the Clinical Details area of the form.
- If a sample is known or suspected to contain group 3 blood-borne pathogens (HIV, Hepatitis B/C) then the request form MUST be annotated "HIGH RISK" prominently in the Clinical Details area of the form.
- If a result is required urgently, then the request form MUST be annotated "URGENT" prominently in the Clinical Details area of the form.
